CareOS is not an EHR.
Once you generate structured clinical data, the pull is to become the system of record. We chose the opposite. CareOS routes into the EHR a facility already trusts.
Every time we ship a new observation type — a fall flag, a meal-intake summary, a behavioral note — someone asks the obvious question. If you already have structured clinical data, why not just be the chart? The pull is real. It is also the wrong move, and we have been clear about that from the start.
A facility's EHR is not a piece of software they can swap. It is a legal record, a billing engine, a compliance audit trail, and an IT contract that took two years to sign. Systems like PointClickCare and MatrixCare are entrenched for a reason — they are integrated with pharmacy, MDS submission, payroll, and the state. Asking a director of nursing to revisit that procurement decision so they can also get better bedside notes is not a trade they want to make.
Where the boundary sits
CareOS owns observation and prompting. The robot is in the room, it hears the visit, it watches the gait, it drafts the note. The EHR owns the legal record — the signed entry, the timestamped chart, the document a surveyor will read. The signature event is the handoff. Before signature, the data lives in CareOS. After signature, it is the EHR's truth.
The interface
Concretely, that means an adapter per vendor. HL7 v2 messages where the EHR still speaks v2, FHIR resources where it speaks FHIR, and vendor-specific REST where the integration team has opened one up. Observation, Encounter, and DocumentReference cover most of what a signed Companion note becomes. Each facility's integration is a known shape — credentials, endpoints, code mappings — not a custom build.
The product impact is what we wanted. Facilities adopt CareOS because it makes the EHR they already paid for more accurate and less painful to feed. No migration, no parallel chart, no procurement fight. The next adapter on the bench is for a regional EHR we keep hearing about in the Southeast, and it ships the same way the others did.
